Limited Power of Attorney - Medical Treatment


Overview
The Limited Power of Attorney - Medical Treatment is used by a parent or legal guardian to authorize medical treatment for their children in their absence (such as long vacations or extended business travel).
The Limited Power of Attorney - Medical Treatment may also be used to give power to another person to act on your behalf should you not be reached in the case of a medical emergency.
Provide a copy of the health insurance, dental insurance and/or prescription card to the individual named in the document to assist them in obtaining treatment for your child. 

When You Need It
-To ensure medical treatment will be given to your child or children according to your wishes in the event you are away from home during a medical emergency.

Getting Started

You will need:
-The name and address of the parents giving the power to provide medical treatment to their child. 
-The name and address of the person or persons who will have the power to make medical decision on behalf of the parents in their absence. 
-The names and dates of birth of all children covered by the document.
-The length of time the document is to be in effect.

When to Review and Revise  
-Desire to change the person or persons making the decisions.
-Adjustments necessary to the directives as a result of additional children or time the document is to be in effect.
