This document can help report an incorrect charge or debt in your name as a result of identity theft.

When You Need It
- Notify creditors about the theft of your identity and assist in the investigation and prosecution of the identity thief.
- Notify creditors about an incorrect charge or debt in your name as a result of identity theft.
- Avoid being responsible for charges and debits incurred by an identity thief.


Getting Started

You will need:
- All your personal contact information, including the account information for the creditor you are contacting. 
- Information about the fraudulent debt or charge incurred using your identity.
- Send each letter to each creditor by certified mail, return receipt requested.

When to Review and Revise 
- To add more detailed information to the document.
- To provide similar information to multiple creditors.
